What do I need to bring at my first visit?

Filled out registration package duly signed, recent medical records, medication bottles or current medication list.

What do I need to bring at subsequent visits?

updated medication list / medication bottles

How do I get medication refills?

It is best to do these during a visit so we can thoroughly review all mediations you are taking including supplements to make sure there are no conflicts. We hesitate to refill medications if a patient is not seen/reviewed within a 6 month period.

Missed appointments

We provide multiple reminders and opportunity for you to cancel a scheduled appointment at least 24 hours prior.  This will enable us adjust our staffing or enable us open the slot for someone that needs to be seen urgently. We ask you respect this as failure to notify us on more than 2 occasions could cause the clinic to discharge you

After hour availability

We encourage you to contact the office during office hours to address urgent concerns since it is better to reference your record and other specialists involved in your care like the pharmacy etc. We understand emergencies do occur and you can reach the physician on call by calling the office number and you will be transferred to the answering service who contacts the physician on your behalf

Referrals

We provide referrals to specialists but limited by those in your insurance plan. When we are unable to obtain one, we will ask you to speak to your insurance to obtain an updated list that we can utilize.

Forms to fill

It is best to bring in the form at an office visit so we can review and fill it out together as appropriate

Do you have on site lab service?

At the moment, we have very limited lab collection in the office but plan to add more comprehensive blood and urine specimen collection in the near future.

Billing issues

Please contact our billing manager at 937-224-4325 to discuss any billing concerns.  We maintain utmost transparency with our billing practices.
